Information You Provide Directly to Us. ProMedica may collect information you provide directly to ProMedica via the Site. This information may include personal information such as contact information (e.g., your first and last name, email address, phone number), job application information, physician practice information (e.g., physician license number), and billing information (e.g., payment card details). Your Choices

Communication preferences and marketing. If you do not wish to receive marketing emails from us, you can opt out of future marketing emails by following the instructions provided in the marketing emails. Opting out of marketing emails will not affect our administrative emails to you (e.g., emails about your transactions).

Accessing, reviewing, and changing your personal information. Registered users can review and change certain personal information (e.g., email address, password, payment information) at any time by accessing their accounts on the Site.
